How to become a member:

1.  Take a tour of Seattle Clubhouse,
    call to schedule. (206) 501-3730 

2.  Complete an application form.   

3.  Have a referral form filled out by your
    mental health provider.

4.  Forms can be faxed to: 206-501-3733 
   or brought by the Clubhouse.​
